Safety Performance History Inquiry

49 CFR 391.23(d)-(e), 391.53 (driver investigation history file)

Within 30 days of hire the carrier must ask every DOT-regulated employer of the past 3 years about the driver's accidents and drug and alcohol history, and keep the responses (or a record of each attempt) in a separate driver investigation history file (391.53), with access limited to the people involved in the hiring decision. Send one copy of this form per previous employer, with the driver's consent below. Record the date sent and the response or follow-up attempts.
